The difference between epileptogenic zone and epileptogenic lesion in the surgical therapy of epilepsy

Failure to differentiate the epileptogenic zone from epileptogenic lesion often leads to violations of the presurgical evaluation protocol in cases of drug resistant symptomatic extratemporal epilepsy. we present a 21 years-old male patient with symptomatic extratemporal epilepsy. he underwent a surgical removal of his epileptogenic lesion a few years ago, without any therapeutic effect. long-term video-eeg recording with intracranial electrodes offered data on the exact delineation of the seizure initiation zone and on the detailed functional mapping of the adjacent cortex as well.The attempted tailored surgical resection based on the data with intracranial recording resulted in freedom from generalized and complex partial seizures.
